MEDICALLY SUPERVISED WEIGHT LOSS PROGRAM

Dr. Aguero has successfully offered this program for the last two years to all his patients.

This program is individually tailored to your specific needs and goals, focusing on safe, sustainable weight loss through a combination of diet, exercise, and medications (oral or injectables, or both).

Our Program Key Components:

1-Personalized Plan: Our Program starts with a comprehensive evaluation and Body composition analysis (BCA). Plans are personalized based on metabolic rate, health conditions (e.g., type 2 diabetes, hypertension), and personal goals.

2-Pay as you Go: No Club or membership charges, nor future credit cards payments. You pay as you go. Weekly, Bi-weekly or Monthly. There is no need to continue payments if you do not tolerate medications well.

3-Dietary Guidance: Dietary daily calories intake based on the results of body composition analysis with informing your Basal Metabolic Rate (BMR), recommendations are given regarding daily caloric expenditure.

4-Physical Activity: Basic increase in metabolic activity/physical activity is always recommended. As a starting point at the baseline of brisk walking for 30 minutes per day, 3 times a week is a good starting point.

5-Medications: Oral phentermine, and injectable GLP-1 agonists (Semaglutide Tirzepatide) may be used to suppress appetite or enhance satiety.

6-Ongoing Monitoring: Regular check-ins (weekly/monthly, with bi-monthly body composition analysis, are needed to track progress, adjust plans, and monitor safety and potential side effects.

7-Medically Supervised Team of Professionals: IN PERSON Doctors and Nurse Practitioners visits, with every 4 weeks follow up, with advice and recommendations to a better and safer medication use (No needing to increase dose in certain cases) and better long-term maintenance.

BODY COMPOSITION ANALYSIS (BCA)

Advanced assessments to track your progress, including fat, muscle, and overall body composition, for a comprehensive view of your health.

Key Measurements:

1-Body Fat Percentage: Measures fat mass relative to total weight per segment. Healthy ranges: 10-20% for men, 20-30% for women (varies by age). High fat increases risks of diabetes or obesity.

2-Muscle Mass: Quantifies muscle weight per segment. Muscle mass naturally declines 50% from ages 20-90, making monitoring critical for mobility and longevity.

3-Body Water Percentage: Total fluid as a percentage of body weight (healthy: 50-65% men, 45-60% women). Tracks hydration changes due to diet, exercise, or medications.

4-Visceral (Truncal) Fat: THIS IS THE MOST RESILIANT FAT TO GET RID OF. Fat around abdominal organs, linked to cardiovascular risk. Measured as a rating (1-59), with ≤12 considered healthy

5-Basal Metabolic Rate (BMR): Calories burned at rest, influenced by muscle mass. Higher muscle mass increases BMR, aiding weight loss. This is a guide for our calories needed in our diet every day.

6-Body Mass Index (BMI): Less precise than body composition but included for reference.

Oral Medications for Appetite Control: Phentermine

-Phentermine is an oral prescription medication used for short-term weight.

It’s a stimulant that suppresses appetite by affecting the central nervous system, like amphetamine.

-How It Works: Increases neurotransmitters (norepinephrine, dopamine) in the brain, reducing hunger. Taken as a daily oral tablet or capsule, often in the morning to avoid insomnia.               

-Short-term effectiveness: Studies show phentermine can lead to 5-10% body weight loss over 12-24 weeks when combined with lifestyle changes. MUST HAVE a BMI > 30. Usually prescribed for 3-6 months. Most common side effectsDry mouth, insomnia, increased heart rate, nervousness, constipation. REQUIRE MONTHLY VISITS FOR REFILLS

Injectables for Appetite Suppression: Semaglutide and Tirzepatide

Semaglutide and Tirzepatide are referred to as GLP-1 ANALOGS (due to their mechanism of action) and work by mimicking the action of naturally occurring hormones. Both work by slowing the digestion process, which can help you to feel full for longer.

 

Some key features and benefits of BOTH include:

1-Blood sugar control

2-Weight management: Both meds have shown to promote weight loss by reducing appetite and increasing feelings of fullness

3-Cardiovascular benefits: Clinical trials have demonstrated that Both meds can reduce the risk of major adverse cardiovascular events

How fast can you lose weight on injectables GLP-1 analogs?

-Weight loss on GLP-1 medications can begin to be noticeable on week #4 . 

-Many of my patients experience a more noticeable decrease in hunger and weight loss between week 6 and 12.  Significant weight loss (up to 15% with Semaglutide and 20% of initial weight with Tirzepatide) is typically achieved over a longer period, such as 20 to 32 weeks, particularly when combined with a healthy diet and exercise.

To be eligible for injectables (Semaglutide/ Tirzepatide) you MUST have:

-No history of Type 1 Diabetes

-No history of Gastroparesis

-No history of Recent Bariatric Surgery (within the past 18 months),

-No history of / Active Gallbladder Disease

-No History of Pancreatitis

-No family history/ nor personal history of Medullary Thyroid Cancer (MTC) or

   Multiple Endocrine Neoplasia Type 2 (MEN2 syndrome)

-NOT being pregnant /intending to become pregnant/ nor breastfeeding
